CHAMPVA, Dental, and Your Family: What Your VA Rating Actually Covers

Veterans at 100% P&T unlock CHAMPVA for their families - but the dental question is more complicated than most expect. Here's how family healthcare actually works at every rating level.

Two questions come up constantly from veterans who've just hit a higher rating: "How do I get my spouse on my insurance?" and "Does 100% mean I finally get free dental?" The answers aren't as straightforward as they should be - and the gaps surprise a lot of families. Let's walk through what your VA rating actually unlocks for your family's healthcare, what CHAMPVA covers (and doesn't), and how dental works for both you and your dependents.

CHAMPVA: Healthcare for Your Family

CHAMPVA - the Civilian Health and Medical Program of the Department of Veterans Affairs - is a health benefits program for the spouse and dependents of certain veterans. To qualify, you need one of these criteria:

  • You have a permanent and total (P&T) disability rating - typically 100% P&T
  • You died from a service-connected disability (your survivors may qualify)
  • You were rated P&T at the time of death

For living veterans, the key distinction is permanent and total. A 100% schedular rating alone isn't always enough - check your benefits summary letter to confirm P&T status.

What CHAMPVA Covers

CHAMPVA is a cost-sharing program with an annual outpatient deductible and typically 25% cost share after the deductible. It covers doctor visits, hospital care, mental health services, prescriptions, preventive care, durable medical equipment, and maternity care. There's also a catastrophic cap that limits total out-of-pocket costs per year.

Important: if your spouse or dependent is eligible for TRICARE, TRICARE is the primary payer and CHAMPVA cannot be used.

What About Ratings Below 100% P&T?

If you're rated below 100% P&T and not a military retiree with TRICARE, the VA does not provide healthcare for your spouse and dependents. Your options are employer-sponsored insurance, ACA Marketplace plans, or Medicaid if income-eligible.

Now the Big One: Dental

VA dental eligibility is entirely separate from VA healthcare eligibility. Being enrolled in VA healthcare does not mean you get dental.

Dental for the Veteran

Here's who gets comprehensive VA dental care:

  • 100% schedular rating or 100% via TDIU: Eligible for comprehensive dental care - cleanings, fillings, crowns, dentures, the works
  • Service-connected dental condition: Eligible for treatment of that condition
  • Former POWs: Eligible for comprehensive dental care
  • Veterans with conditions aggravated by dental problems: May receive dental treatment if complicating treatment of a medical condition

If you're rated 90% or below without a service-connected dental condition, you're generally not eligible for VA dental care.

Veterans who recently separated can apply for VA dental care within 180 days of discharge - don't wait, the deadline is firm.

Dental for Your Spouse and Dependents

CHAMPVA does not cover routine dental care for your family. No cleanings, no fillings, no crowns. There are narrow exceptions for dental care medically necessary as part of a covered condition, but routine family dental is excluded.

Your family dental options are:

  • VA Dental Insurance Program (VADIP): Discounted insurance available through VA, but covers veteran only
  • Private dental insurance: Through employer or purchased individually
  • Dental discount plans: Can reduce costs at participating providers
  • Community health centers: Often provide reduced-cost dental care

Putting It All Together

  • Below 100%: No family healthcare through VA. Veteran gets VA medical care but generally no dental. VADIP available for veteran dental insurance.
  • 100% (not P&T): Veteran gets comprehensive VA dental. Family CHAMPVA eligibility may be delayed until P&T status confirmed.
  • 100% P&T: Veteran gets comprehensive VA dental. Family gets CHAMPVA for medical care. Family dental is still not covered.

How to Apply for CHAMPVA

Submit VA Form 10-10d with copies of dependents' information and your rating decision letter confirming P&T status. Processing can take several weeks - start immediately upon receiving P&T determination and keep medical receipts, as CHAMPVA can sometimes apply retroactively.
